This book addresses an urgent need, offering an updated, detailed and multidisciplinary review of surgery of the inferior vena cava (IVC). Over the past two decades, tremendous advances have been made in this field and related specialties. Many journals and/or textbooks on different surgical or medical specialties have reported on individual aspects, so it is high time to gather all of them in a single resource. To date, surgery of the IVC remains a major challenge, not only in terms of technical difficulties, but also in terms of perioperative management. The book sheds new light on cutting-edge developments, including: Innovations in imaging; Neoadjuvant and adjuvant treatments for tumors of or involving the IVC; Intraoperative anesthetic management; and Technical and technological advances in surgery. As such, it will be of a great interest to surgeons (cardiac, vascular and digestive) and urologists, as well as radiologists and anesthesiologists.

This revised, updated and expanded edition takes account of all the recent developments in these areas. New chapters on, for example, foam sclerotherapathy, radiofrequency treatment, laser treatment and open surgical reconstructions are included, as well as useful diagnostic and treatment algorithms for the various conditions that are dealt with in the book. Clinical guidelines are provided in each chapter, together with evidence scores to help the reader assess the recommendations. The Handbook of Venous Disorders is written and edited by leaders and founding members of the American Venous Forum, a society dedicated to research, education and the clinical practice of venous and lymphatic diseases. The Handbook also includes several international authors, all of whom are experts in venous disease, most being regular or honorary members of the American Venous Forum.

This book shows how new technologies and technical skills together with deeper understanding of pathophysiology of visceral and renal ischemia have contributed to significant improvements in the clinical outcomes in patients undergoing complex aortic repair involving splanchnic arteries. In recent years, aortic repair has expanded its borders, focusing more and more on the particularly challenging segments from which critical branches originate. Optimal results in this area are obtained through a multidisciplinary approach based on crucial elements such as sophisticated imaging techniques, advanced anesthesiological and pharmacological strategies, as well as updated surgical techniques and devices able to reduce ischemic injuries. The book presents problems tackled from different perspectives, by analyzing the main technical aspects of the surgical technique but also functional and metabolic issues. It also discusses concepts such as ischemic preconditioning and ischemia-reperfusion mechanisms, as well as innovative investigational approaches. In the “endovascular era”, it highlights the most recent updates in complex endovascular repair involving renal and visceral arteries, providing both practical tips and tricks and outcomes analyses. The volume also addresses specific, rarely reported problems in vascular surgery, including issues in renal transplant, oncologic surgery with involvement of splanchnic arteries and of the vena cava, and use of the deep veins in aortic surgery. This volume represents a powerful tool for both young and experienced operators who wish to approach the complex aortic pathology either by open surgery or through endovascular methods.”

This book addresses placement and retrieval of inferior vena cava (IVC) filters. Until 2010, utilization of IVC filters had been increasing since their introduction in 1967. Studies in the early 2000’s, however, identified that prolonged caval interruption with IVC filters were fraught with complications including filter fracture, migration, and caval thrombosis. This subsequently led the FDA to issue an advisory recommending judicious placement and timely retrieval of IVC filters by treating physicians. This safety advisory ultimately created a heightened awareness by medical professionals and the general public regarding the negative consequences surrounding long-term caval filtration. Subsequently, IVC filter placements decreased nationally while retrievals conversely increased in the post-FDA advisory era. During this same time period, interest in complex IVC filter retrieval also increased as a method to manage patients identified to have or be at risk for complications secondary to prolonged filter implantation. Given these established trends and interest surrounding advanced techniques in IVC filter retrieval, this book addresses these topics in a reader-friendly, case-based format. Chapters focus particularly around the recognition and management of filter-related complications. Additionally, advanced techniques employed by experienced operators for complex filter retrieval are also discussed. For completeness, the book also includes a review of indications and appropriate methods for IVC filter placement. This book is unique in that, at present, descriptions of complications and advanced techniques utilized for complicated IVC filter removal consist mainly of case reports and case series scattered throughout the literature. This publication serves to compile these sources into a single comprehensive entity for physicians treating patients with IVC filters. Chapters are organized to begin with a few introductory paragraphs highlighting the relevant literature (and providing references for in-depth reading) followed by several cases demonstrating tips, tricks, and procedural pitfalls. When possible, each case will include a step-by-step description of the technique being described. This is an ideal guide for interventional radiologists, interventional cardiologists, and vascular surgeons that perform or are interested in performing filter retrieval beyond standard techniques.
