Knowledge of cutaneous lymphomas has been growing significantly as a result of important discoveries in immunology, molecular biology, and immunohistochemistry. Improved clinical pathologic correlation and follow-up data, as well as the synergistic collaboration among different lymphoma registries and specialists from several academic medical centers have greatly contributed to the understanding of the difficult field of cutaneous lymphoproliferative disorders. While these advances have increased understanding of skin lymphomas, they have also produced an extensive and sometimes confusing litany of articles, studies, and classification schemes. This issue on Cutaneous Lymphomas in Surgical Pathology Clinics provides an organized and updated review of this challenging topic by leading experts. It bridges critical knowledge gaps in the diagnosis of cutaneous lymphomas. Sezary Syndrome, Mycosis Fungoides and variants are presented along with B-cell, CD30, lymphoproliferative disorders among others. In addition to multiple clinical and microscopic images, tables and algorithms are presented to aid in diagnosis and staging. Beyond its usefulness to general pathologists, dermatopathologists, and hematopathologists, this information is intended to be helpful for dermatologists, hematologists/oncologists, fellows, and residents.

The Editor of this publication is Dr Elise Olsen, Director of the Duke University Cutaneous Lymphoma Research and Treatment Center and professor of Dermatology and Oncology. In 2014, cutaneous lymphoma (CL) is considered a chronic, non-life threatening disease since the majority of patients are diagnosed at early stage, though progression to later stages does result in death from the disease. This issue covers T-cell and B-cell lymphomas, with content emphasis on Mycoides fungoides and Sezary syndrome, the two most common clinically encountered subtypes of CL. Pharmaceutical treatment of the disease is a part of each article and the issue uses the following Treatment outline: Mechanisms of action - Pharmacokinetics - Typical dosing - Response to therapy - Adverse effects - and Pearls to help management using the agent under discussion. Beyond Dermatologists, specialists who would find the information useful are Cancer specialists, Hematologists/Oncologists, Physician Assistants, Nurse Practitioners, and Pharmacists. Some of the topics, written by the word's top experts in this disease, are: Overview of primary cutaneous lymphomas and the applicable staging and classification; Pathologic diagnosis of cutaneous lymphomas; in the section on Mycosis Fungoides and Sezary Syndrome, the clinician and researcher will find such topics as Skin directed medications; Phototherapy; Systemic retinoids; Interferons; HDAC inhibiotrs, Methotrexate and praletexate, along with other chemotherapeutic agents. Also, Bone marrow transplant; Diagnosis and management of CD30+ Lymphoproliferative disorders; and Diagnosis and management of primary cutaneous B cell lymphomas.

This atlas contains excellent clinical and histopathologic images and text of each of the types of cutaneous lymphoma (around 25 entities). It is the first go-to text for those who are considering a diagnosis of cutaneous lymphoma in their differential diagnosis. The text also includes diagnostic mimics of lymphoma and differential diagnosis tables and algorithms. The target audience is general practitioners, dermatologists, pathologists and students, residents and fellows. The diagnosis of lymphoma in the skin is confounded by the myriad of disorders that can mimic lymphoma clinically and histopathologically and by inconsistencies in the diagnostic classification that have only recently been resolved. In the last decade the European Organization for Research and Treatment of Cancer (EORTC) Cutaneous Lymphoma Group and the World Health Organization (WHO) Collaborated in a series of workshops and consensus meetings to arrive at a definitive classification scheme for cutaneous lymphoma. Unfortunately, the publication by the WHO that described this schema included all lymphomas and has the skin tumors scattered throughout the volume. There is currently no go to text for those who are considering a diagnosis of cutaneous lymphoma in their differential diagnosis. As a result there continues to be confusion about the diagnosis of cutaneous lymphoma, although this classification scheme was published in 2008.

This issue of Surgical Pathology Clinics is devoted to Soft Tissue Tumors, the first in this series was presented in 2011. This issue addresses the most difficult diagnostic challenges and focuses on differential diagnosis in soft tissue tumors. Each presentation is accompanied by abundant histologic slides to display the diagnostic differences. Additionally, authors selected two to five diagnoses they find can be particularly difficult, with an emphasis on how to approach such lesions on biopsy samples where relevant and the role of ancillary studies. Topics include coverage of diagnostically challenging: Vascular lesions; Retroperitoneal “Fatty tumors of adults; Smooth muscle neoplasms; Chondro-osseous lesions of soft tissue; Pediatric tumors; Epithelioid tumors; Spindle cell neoplasms of the retroperitoneum; and Peripheral nerve sheath tumors. Also presented are: Non-mesenchymal mimics of benign and malignant soft tissue tumors; Soft tissue tumors with overlapping molecular findings; Recently characterized soft tissue tumors; Benign mimics of sarcoma; Advances in molecular methods in the analysis of soft tissue tumors and therapeutic implications; and Myoepithelial tumors: an update. Leona Doyle and Karen Fritchie lead this issue of experts in soft tissue pathology.
