Excerpt from The Journal of Biological Chemistry, 1907, Vol. 2 I have thought it worth while to bring together here a number of observations relating to bacterial processes in the intestinal tracts of persons suffering from severe forms of anaemia, in the belief that they afford a clue to the etiology of many cases of advanced blood disease that are ordinarily described as "primary." Most of the instances which form the basis of this study have given the blood-picture of so-called idiopathic, pernicious anaemia. The observations relate especially to seventeen cases of anaemia. Of these nine showed those changes in the morphology of the red cells (including the presence of megaloblastic forms) which are regarded as most characteristic of primary pernicious anaemia (Cases I, III, V, VII, VIII, X, XII, XIII, XVII). In four other cases (Cases II, IV, VI, and IX) the diagnosis of pernicious anaemia was made but appears less certain as megaloblasts were not seen. Of the remaining four cases one is an adult from whose history and blood-picture a distinguished consultant leaned to the diagnosis of pernicious anaemia, while another practitioner, upon the same evidence, suspected that he was dealing with an anaemia secondary to gastric carcinoma (XV). Another case is one of severe anaemia which was regarded as secondary, but for which no explanation could be found at autopsy (Case XI). Another case is that of a young child in the care of Dr. Holt, whose history is especially instructive as showing the rapidity of the process of blood regeneration upon the subsidence of a subacute intestinal process to which the anaemia was apparently secondary (Case XVI).
