New Trends In Cerebral Aneurysm Management

The management of cerebral aneurysms is still the subject of controversy in spite of recent dramatic advances in surgical techniques and neuro-intensive care. Currently, two main topics in this field have totally different aspects: 1. the management of unruptured cerebral aneurysms as a preventive medicine and 2. neurocrutical management of severe subarachnoid hemorrhage (SAH) after the rupture. These topics were discussed during the Swiss-Japanese Joint Conference in Zurich, Switzerland, in May 2001. Selected papers from this meeting are included in this volume presenting new clinical experiences in order to find out updated and proper ways to focus the treatment and providing updated information on neurocritical care aspects as well as endovascular and surgical treatment modalities carried out in daily practice in Zurich and Japan.

Professors Da Pian and Pasqualin deserve high compliment and it is a measure of the respect in which they are held that virtually all those most knowledgeable around the world attended, presented their work and thoughts and contributed to intense discussion. Subarachnoid hemorrhage from aneurysm rupture still constitutes, I dare say, the most difficult problem for neurosurgeons, in relief of the brain injury and arterial reaction and the technical perfection of aneurysm obliteration, even for small, as well as large and giant sacs. Very large high flow A VMs can be as demanding too. The bulk of the conference was devoted to subarachnoid hemorrhage, aneurysms and A VMs which were discussed under about 14 headings each. But vein of Galen malformations, dural A VMs, cavernous angiomas and venous angiomas (renamed developmental venous anomalies) came under scrutiny, not always with consensus. Trends are perceptible such as fibrinolysis of subarachnoid clot, non surgery for Galenic and dural malforma tions, the benignity of venous angioma, but there is still much variation in approach, pharmacologically, technically and with such as the evolving endovascular and radiosurgical stories, used alone or in conjunction.

In July 2004 specialists in neurosurgery, neuroradiology, neurology and neurointensive care discussed recent trends at the 2nd Swiss Japanese Joint Conference on Cerebral Stroke Surgery, held in Zurich, Switzerland. New concepts were worked out during the conference and are published in this volume. The book starts with the topic intracranial aneurysms, discussing microsurgical and endovascular treatment modalities, as well as new surgical approaches. Further chapters deal with the management of unruptured aneurysms and with subarachnoid hemorrhage. Practical guidelines for vasospasm treatment are given. Together with contributions about arteriovenous malformations and fistulas, cerebral revascularization techniques and surgery related to the intracranial venous system a comprehensive overview about stroke surgery is given with an interdisciplinary approach.
