Annotation. This publication presents new and fascinating insights especially into the pathogenesis of both acute and chronic pancreatitis. It starts with a literature review and presentation of the authors¿ own data on laboratory markers of acute pancreatitis such as trypsinogen, serum amyloid A and procalcitonin, paying special attention to their clinical usefulness and test performance for stratifying severity and monitoring disease progression. Also discussed is the still difficult differential diagnosis between pancreatic cancer and chronic pancreatitis, especially the early diagnosis of pancreatic cancer in preexisting chronic pancreatitis: particularly, the high specificity of molecular alterations in pancreatic cancer as found in some pilot studies has the potential of a strong complementary marker of malignancy in patients with a pancreatic mass of uncertain origin. Further topics discussed are acute-phase reactions during early pancreatic cellular injury, tropic pancreatitis, the pathogenesis of pain in chronic pancreatitis and the pathomechanisms of pancreatic fibrogenesis and the role of pancreatic stellate cells in the fibrotic process. Last but not least, endoscopic therapies in chronic pancreatitis are looked into.

This book aims to raise awareness of IgG4-related sclerosing cholangitis among practicing physicians and to equip readers with a sound understanding of the principles of diagnosis and treatment. Clinical, serological, and histopathological features are clearly described and imaging appearances on all relevant modalities are illustrated, covering the bile duct and other involved organs. Differential diagnosis from other diseases, including primary sclerosing cholangitis and cholangiocarcinoma, is precisely explained. Information is then presented on all significant current and emerging therapeutic strategies, including steroids, immunosuppressive drugs, and rituximab. Finally, attention is drawn to significant prognostic features. While IgG4-related sclerosing cholangitis is now a widely acknowledged condition, most practitioners are still liable to misdiagnose it owing to a lack of familiarity with its presenting features. This book should help to rectify the situation and will be an asset for all who may encounter the disease in clinical practice.

Pancreatic diseases are a crucial issue not only because of their epidemiological significance, and their anatomical and metabolic complexity, but also because of the different treatment options available and their implications for surgery. Due to the slow but steady increase in the incidence of pancreatitis and pancreatic cancer, these diseases are today a real challenge for the surgeon. This volume presents the state of the art of surgery in the treatment of inflammatory and neoplastic pancreatic diseases, describing in detail the different approaches and their most relevant technical aspects. Also described are conventional and interventional radiology, operative endoscopy and endoscopic ultrasonography (which has recently acquired a diagnostic role), and adjuvant and neoadjuvant therapy for treatment or palliation: for only a multidisciplinary approach can produce a global vision, a winning strategy for the optimization of results. Along with the texts, all written by experts, the volume includes color images and drawings. It will prove itself a very useful tool in clinical practice.
