In Vitro Three Dimensional Biomechanical Comparison Of Two Internal Fixation Methods In Equine Adult Radii

Radius fracture configurations conducive to internal fixation most often contain a spiral or oblique fracture of the distal diaphysis. The location and fracture configurations are biomechanically challenging because of the limited amount of bone available distally for screw purchase and the complex three dimensional (3D) loading and unconstrained motions that occur in the distal radius. Development of an in vitro loading-measurement system that mimics in vivo unconstrained 3D relative motion of long bones, applies uniform load components over the entire length of a test specimen, and measures 3D relative motion to directly determine construct stiffness was verified. Stiffness results were comparable in magnitude to those theoretically predicted, and were consistently higher than results in the literature due to elimination of potting-fixture-test machine finite stiffness. Construct failure configurations were always reproducible with theoretical failure modes for bone. Biomechanical properties of the dynamic condylar screw (DCS) implant system and the double broad dynamic compression plate (bDCP) construct used to repair distal oblique diaphyseal osteotomies and ostectomies in adult cadaveric radii were compared. No statistical difference was observed between the DCS implant system and the bDCP construct stiffness during axial compression, torsion, or four-point bending; intact radii displayed the greatest stiffness. Torsion and four-point bending failure loads were not statistically different for the DCS implant system when compared to the bDCP construct.

External fixation is now being used widely to maintain fractures, osteo tomies, and arthrodeses in a desired position during consolidation. Whereas external fixation has been readily accepted in European countries, its use has weathered a rather stormy course in North America, especially in the treatment of fractures. Only recently has external fixation found its rightful place on this continent as well. Many different models are on the market today, and the practitioner is faced with a difficult decision in selecting a model. Should he buy a system where the fracture has to be reduced first, or should he work with a device permitting a reduction after insertion of the pins? To enable surgeons to study the different systems, to discuss their advantages and disadvantages, and to permit them to put their hands on these devices and inspect them personally, the Division of Orthopedic Surgery, University of Ottawa organized an applied basic science course in May 1981, External Fixation of Fractures. During this course, all major systems were pre sented to the participants. As happened during the course "Internal Fixation of Fractures" held two years ago, the rigidity of internal fixation was frequently and intensively debated. Whereas the rigidity of internal fixation cannot be altered during the course of healing, the rigidity of external fixation can be changed. In fact, with progression of union, rods of increasing elasticity can be used.

After the publication of the AO book Technique of Internal Fixation of Fractures (Miiller, Allgower and Willenegger, Springer-Verlag, 1965), the authors decided after considerable discussion amongst themselves and other members of the Swiss AO that the next edition would appear in three volumes. In 1969, the first volume was published (the English edition, Manual of Internal Fixation, appeared in 1970). This was a manual of surgical technique which discussed implants and instruments and in which the problems of internal fixation were presented schematically without radio logical illustrations. The second volume was to be a treatise on the biomechanical basis of internal fixation as elucidated by the work done in the laboratory for experi mental surgery in Davos. The third volume was planned as the culminating effort based upon the first two volumes, treating the problems of specific fractures and richly illustrated with clinical and radiological examples. It was also to discuss results of treatment, comparing the results obtained with the AO method with other methods. The second and third volumes were never published. The second edition of the AO Manual appeared in 1977. It dealt in greater detail with the problems discussed in the first edition, although it still lacked clinical exam ples and any discussion of indications for surgery. Like the first edition, it was trans lated into many languages and was well received. Finally, after 22 years, the much discussed and much needed third volume has appeared.
